A respected not-for-profit organisation is seeking a  Community Support Facilitator  to join their team and make a meaningful difference in the lives of community members.

About the Role
As a Community Support Facilitator, you will work closely with community members, providing practical support, advocacy, referrals, and assistance to help individuals navigate services and improve their wellbeing. You will play a key role in fostering inclusion, community participation, and positive social outcomes.

Key Responsibilities
  • Provide person-centred support to community members with diverse needs
  • Assist individuals to access appropriate community, health, and support services
  • Conduct intake, assessment, referral, and follow-up activities
  • Maintain accurate records, case notes, and documentation
  • Build strong relationships with stakeholders, service providers, and community networks
  • Support community engagement initiatives and programs
  • Ensure service delivery aligns with organisational policies and funding requirements
About You
To be successful in this role, you will have:
  • Experience within community services, disability, mental health, aged care, family support, or a related sector
  • Strong interpersonal and communication skills
  • The ability to engage effectively with people from diverse backgrounds
  • Excellent organisational and administrative skills
  • Experience maintaining client records and managing documentation
  • A proactive and collaborative approach to work
  • Relevant qualifications in Community Services, Social Work, Human Services, or a related discipline (desirable)
