Service Coordinator - Aged Care Services
  
Beaumont People are currently recruiting two newly created roles, we  looking for a passionate Service Coordinators to work within Age Community Care services. These are full time, permanent position located in Parramatta in a hybrid work from home and office environment.
  
As Service Coordinator you will coordinate the organisations Aged Care Services undertaking CHSP client care planning, monitoring and reviews. While also provide administration support and resolving client requests within applicable service levels.
  
You will be a strong and open leader, building positive relations with a team of Support Workers reporting to you. Working across divers programs you will be responsible for the implementation and daily operations of the in home, centre based day respite, community visitors scheme and community support programs.
  
You will be responsible for working with people to implement a person centred service delivery model to older adults and people with disabilities through:

In this role you will;
  • Liaise with clients and their carer / family to development and implement client driven care plans.
  • Support and promote the individual needs of clients to maximise their goals and ensuring quality supports
  • Lead, manage and support a team of staff and volunteers working in centre based and community settings.
  • Maintain client expectations by providing timely response to all enquiries via phone, email, portal or other designated tools
  • Identify and appropriately escalate client risk, health & wellbeing matters in accordance with authorised processes and quality standards
  • Ensuring the delivery of services meet our contracted service outputs and outcomes
  • Networking with external providers including Regional Assessment Services, Allied Health and agency meetings.
  • Managing client and carer's needs assessments, reassessments, and regular care plan reviews 
A bit about you

To have success in the role you will be a confident communicator who is highly organised, has excellent time management with a keen eye for detail. You will enjoy working in a collaborative and solution focused environment that focuses of high-quality services to people in the community.

You will also have;
  • Experience within a Coordinator role within the Aged Care industry along with knowledge of the functional impairments experienced by older adults and people with a disability
  • Solid understanding of relevant standards and legislation related to Aged Care
  • Experience developing client care and support plans in a person centred frame work, to maximise their goals and ensuring quality support to our clients
  • Well developed interpersonal skills with the ability to establish rapport and communicate effectively with clients and their families / carers and network at a local and inter-agency level
  • Proficient in relevant computer applications, and data entry. As well as an ability to learn and adapt to new computer systems
  • Have a valid Working with children check and be open to undertaking pre employment checks including NDIS Worker and criminal history check
  • Availability to be on call, as per roster – Paid allowance is available

The annual base salary for this position is on a SCHADS level 4 starting from $72,000 - $78,000 + employer's contribution to superannuation and NFP fringe benefits.
