Case Manager - Mental health and complex support services 
Beaumont People are currently recruiting for Case Manager for a program providing shared transitional accommodation for people living with complex mental health, previous trauma and who may also be living with a disability.
The focus of the program is to work with participants assisting with psychosocial support needs, carrying out assessments around individual support plans, risk assessments and safety plans. You will also work in a holistic approach to proving case management services empowering participants to shrive towards goals, connect to community services and assistance to obtain permanent accommodation.
  • 6 month initial contract, to start ASAP

Main duties of the role are;
  • Ensure participants have Individual Support Plans, Risk Assessments, Safety Plans and One Page Profiles that meet quality standards and are inline with participants goals and life plans. 
  • Work closely as part of a multidisciplinary team to conduct regular reviews with key stakeholders and participants to monitor progress against the plan and ensure plans are responsive to and ever changing environment.
  • Respond to incident notifications and develop plans to appropriately respond to emerging trends or behaviours of concern 
  • Complete SIL profiles and compile supporting information for submission to NDIS 
  • Liaise with external providers to share information, facilitate holistic care planning and support the integration of support services 
A bit about you;
To have success in this role you will be willing to hit the ground running and work
  • Relevant qualification in Social Work or other similar areas with advanced Case Management and assessment skills 
  • Experience working within trauma informed, strength based practice frameworks with a understanding of the complex issues faced by people living with a mental health conditions and /or have experienced homelessness
  • Build and maintain strong and respectful relationships with participants, internal and external community stakeholders.
  • Have strong and effective communication skills, both written and verbal along with good computer literacy skills.
  • Hold a unrestricted NSW driver's licence, Current police check, first aid certificate and NSW Driver’s license.
