Job Description

Main Responsibilities
  • Take inquiries and referrals, conduct assessments, prepare care plans and monitor and address client needs in consultation with the Bilingual Support staff (or interpreters) and other relevant agencies.
  • Make referrals to other services to meet the needs of the clients
  • Provide an ongoing case management service to clients with care needs.
  • Monitor and review clients’ needs to ensure the effective provision of services so that the appropriate support services continue to be provided.
  • Keep in regular contact with relevant services eg, ACAT, Aged Care and ethno-specific agencies.
  • Monitor and review client base periodically and develop improvement strategies using service and demographic data.
  • Keep accurate and up to date client files, lists and data bases
  • Exhibits a concern for excellence in service delivery to older people (including those living with dementia), their families and their carers
  • Ensures priorities and objectives for the service are clearly communicated
  • Demonstrates good planning and monitoring skills
  • Makes sure good systems are in place to support effective service delivery
To be successful in this role you will have:- 
  • Appropriate tertiary qualification in Social Sciences and/or experience in similar capacity;
  • Excellent Case Management Skills;
  • Demonstrated understanding of needs and issues of older people from CALD background and their carers;
  • Demonstrated Experience in needs assessment and management of older people;
  • Excellent communication skills;
