Job Description

Beaumont People are excited to partner with Spinal Cord Injuries Australia who are recruiting for Community Services Coordinator in Alstonville on the Northern Rivers.
  
In a newly created role due to growth the Community Services Coordinator will assist clients with planning for services, referrals and coordination of services and supports, primarily under the NDIS to people with a spinal cord injury (or similar physical disabilities), their families and carers 

As part of the role you will be required to establish and developed good relationships with other organisations, allied health practitioners in the spinal units and community to promote issues that affect people with spinal cord injuries (or similar physical disabilities)

The Community Services Coordinator as part of their everyday tasks will undertake a number of duties including;  
  • Providing case management to customers
  • Providing direct support coordination and plan management/financial intermediary services for our NDIS customers
  • Attending NDIS planning meetings and liaising with NDIS planners as required
  • Creating and completing goal setting and outcome based plans for customers
  • Collaborating with customers, families, providers, medical practitioners and other stakeholders.
Experience you will need to have;
  • Case management experience along with a sound understanding of the NDIS participant plans
  • Strong analytical, problem solving and conceptual skills
  • Good financial acumen including the ability to review and explain financial statements
  • Have a person-centred approach with proven ability to work positively with people to reach their individual goals
  • Excellent documentation skills with records and incident planning and reporting
  • The ability to meet KPI's in relation to billable hours
  • Unrestricted NSW driver's licence and be open to undertaking pre employment checks. 
Ideally you will also have;
  • Industry relevant formal qualifications, in Disability or Community services
